Does Insurance Cover a Home Birth Midwife? (Superbills Explained)

Short answer: most home birth midwives — including Wild Moon Midwifery, a cash-pay practice serving Leland, Wilmington, and throughout Brunswick and New Hanover Counties, NC — don't bill insurance directly. But that doesn't mean insurance can't help cover the cost. The tool that makes this possible is called a superbill.
What is a superbill?
A superbill is an itemized receipt your midwife provides after you've paid for care, listing the services provided along with the medical billing codes (CPT and ICD-10 codes) insurance companies use to process claims. You submit it yourself, directly to your insurance company, as an out-of-network claim once baby is born.
How the reimbursement process actually works
You pay your midwife directly for care (this is the "cash-pay" part).
Your midwife provides an itemized superbill once care is complete.
You submit the superbill to your insurance company as an out-of-network claim — usually through their member portal or by mail.
Your insurance company reviews the claim against your specific plan's out-of-network maternity benefits.
If approved, they reimburse you directly — not the midwife.
Will you actually get reimbursed?
This is the part that's genuinely plan-dependent, and no midwife can promise a number upfront. Reimbursement depends on:
Whether your plan has out-of-network benefits at all (HMO plans often don't)
Your out-of-network deductible, which is usually separate from — and higher than — your in-network deductible
Whether your plan classifies midwifery care as a covered maternity benefit
The specific CPT codes used and how your insurer processes them
Some families are reimbursed a significant portion of their midwife's fee. Others find their plan offers little to no out-of-network benefit. The only reliable way to know is to call your insurance company directly and ask specifically about out-of-network maternity/midwifery reimbursement, ideally before you commit to care.
Questions worth asking your insurance company
"Do I have out-of-network benefits for maternity care?"
"What's my out-of-network deductible, and has any of it been met this year?"
"Do you reimburse for services billed under CPT codes for midwifery/home birth attendance?" (your midwife at Wild Moon Midwifery can give you the exact codes to ask about)
Do my in-network and out-of-network deductible and out-of-pocket maximum accumulate?
HSA and FSA funds
Separately from insurance reimbursement, midwifery care generally qualifies as an eligible expense under HSA and FSA accounts, which is a more predictable way to offset cost since it doesn't depend on your insurer's decision.
